I agree with pat gima here. possible yeast. i see it more and more. you can treat with gv both mom and baby. 
here is what i have had success with is calendula salve on the nipple and 5-10 drops of homeopathic castor equi 30C every 2-4 hours while awake. works really fast and well.  the castor equi may only be needed for a couple of days.
