Hi Sarah
This is your baby.  And this is your body.  You can refuse treatment or
testing.  As a physician, you are in a reasonably good position to be able
to explain that you understand the risks associated with refusing
testing/treatment (with infant formula or HMF) and you are prepared to
accept those risks.  
Do you trust your milk? Have you ever seen any evidence to suggest that it
might present more of a risk to your baby than milk sourced from a cow and
modified in a factory? Stand your ground.
I am sorry to hear about your ruptured membranes.  In Australia, mothers
with ruptured membranes are hospitalised for monitoring.  Their temp is
monitored second hourly and so is their liquor. The baby is also monitored
with regular CTG and biophysical profile (external ultrasound). If the
liquor remains clear and there is no evidence of infection (fever) the
mother is left alone to gestate for as long as she can.  (Also given
hydrocortisone IM.)  The risks associated with preterm delivery (and it
sounds like C-section is routine in your situation there) must be weighed
against the risks of  continued gestation.
