I too agree with your assessment that flow is the problem for this baby.
A few things that have often worked with babies with this problem in my 
practice:
-have mom lay baby on mom's chest, so she can self attach.
-use distraction - show her toys, make funny noises, try to use her 
instincts to help her take the breast when her "guard is down". (I hope 
you know that idiom).
-advise mom never to hold or press the baby's head when she is on or 
near the breast. The baby needs to feel like she can let go when ever 
she needs to. Sometimes just teaching moms this was enough.

Baby can also be checked for a tongue tie or tight lingual frenulum. 
Babies with tongue tie have a harder time handling milk flow, and 
improve once treated.

Mom is angry with you because she idealized you, not because you deserve 
her anger. Some people are dependent types, and "latch on" to helpers, 
then get very angry when they feel abandoned for any reason. If this 
behavior is a regular pattern and is severe, mom may have a personality 
disorder. If she does, set your boundaries (let her know clearly when 
she can call you and how many times a day or week), and try not to make 
any statements that could be construed as promises.
