The AAP guidelines do not call for stopping breastfeeding for jaundice
even under lites. These Docs need to review these practices. Lois

I see no contraindication to pumping. It seems she did so with the last
lactation. I think she may have more engorgement due to induction and
c/s
birth and all the interventions and length of time that involves. So  I
would use reverse pressure softening, cold compresses, and hand
expression
until milk flows better. I would speak to the pediatrician, let him know
that the baby will be getting only a little breastmilk at this point,
and
could mother put him to breast, as she is really suffering with the
engorgement. or at the very least, ask how soon can she resume nursing,
becoz of her problems and strong desire to bf.
Unfortunately, folks, there are still pediatricians who order no
breastfeeding and convince the mother it is dangerous, or at least that
formula is preferable during a bout of hyperbilirubinemia. While I have
seen
a couple of severe cases in which this tactic might be justified *for a
short time* when baby's levels are dangerously high, most of the time,
this
is totally counterproductive and causes premature weaning. See archives
for
previous discussions about formula knocking down the bili levels faster
and
not just becoz of the volume. Just last week, had a term infant, about 3
days old, with a level of 17 who had lots of bruising, and was taken off
breast and put on home photottherapy. If it was so serious it would not
be
on home photottherapy. Oh well I try to work around it, and asked mom to
express in the meantime, save the milk, and resume asap.
Hope the "BFAR mom" hangs in there.
