Are you talking about lactation documentation?  Whatever hospital electronic documentation system that is used should have a lactation -specfic portion for lc's to document their activities AND a nursing section where nurses document what they do.  These should "speak" to each other and flow into a central area where it's easy to see summarized what is happening with the baby and mom.  It's great when it works well.

Subject: hospital flow chart

I am looking for a flow chart that can be used by nurses in a hospital setting.  
I looked through the archives and see some conversation awhile back about this subject but didn't see any link or post of one that might be available.
If someone has such a flow chart I would be interested in receiving that information If there isn't one then I will work on putting one together and would also welcome ideas for a flow chart.
