Whomever stated that the AAP recommends any such thing is wildly mistaken
unless they know about some statement that supercedes the Feb 2005 Policy
Statement "Breastfeeding and the Use of Human Milk".  That statement is
very clear about supplements:

"Supplements (water, glucose water, formula, and other fluids) should not
be given to breastfeeding newborn infants unless ordered by a physician
when a medical indication exists."

I certainly would not interpret "transitioning from the nursery to home"
as a medical indication for ordering supplements.  :/
