I was contacted by a midwife whose patient is nursing a three year old.
During the last three months the mother has experienced breast pain on one
side when nursing and tenderness when that side is touched. She has ruled
out mastitis, (but is willling to treat as though it is a sub-acute
infection) feels no masses or changes in that side, the mother can't recall
any injury, and there are no other signs of yeast in mother or child. The only
change seems to be this constant pain during nursing.

I suggested try to watch over stretching( as though you can convince a
toddler of anything) or her body position during nursing. The mother doesn't
want to wean from that side for even a short time so that isn't an option.

Suggestions please?
