I've been in private practice a long time (20 yrs) and don't wear  gloves. Obviously I wash my hands a lot and use hand sanitizer which I believe is enough.
Of course I  wear a finger cot for oral/suck exam.  I do touch my clients  breasts (with permission)  and their baby.  Never had a problem or complaint  and don't anticipate ever having any.  I think wearing gloves would change the whole "feel" of my practice-
