mom is Brazilian 30 yrs old 1st baby del at 33 weeks 3# mom with positive
Tb past hx of INH therapy premie is home on INH pumped milk and premie ABM,
mom has language barriers so do i and she is hard to convince she has
enough milk. the hospital gave her the Depo shot day 1 pp she had good milk
pumpable 1st 2 weeks 2 oz q 3 hrs but is experiencing diminished supply per
015 model bilat pumping.....baby gained 8 oz in 7 days and i've gotten the
baby to breast for most feeds but seems a bit on the fussy side... massive
orange/yeallow stools 6-8 voids (pooped 3 time in 2 hours while i was at
house) during feeds..
dad is highly reactive Tb with a value of 14 ??????is this sputum culture
colonies ?  2 days ago i saw bright red streak of blood looked like fissure
blood to me when she was feeding 1/2 Abm 1/2 EBM i told her to try to feed
exclusive EBM or Brf baby for 2 days, i tested stool for occult blood and
it was negative whew ! babe sees health dept doc tomorrow.... also a
loosely attached tongue tie noted baby extends nicely great latch, but mom
c/o latch-on pain every time (gets better) baby gets sleepy after about 3-5
mins of sucking and swallows we tried switch nrsing and it worked go 20 =
mins of active nursing mom demonstrates great mer we can hand express with
spray but it diminishes output after 5-6 mins very sensitive to her
"worried nature" naive but loving and is scared to handle infant avoids
burping  or moving baby much handles like glass but kisses and talks to
baby alot needs lots of permission to "mother" her baby.
        I have several questions:

1. What precautions should i use around the Tb positive family i wash well
etc but gloves at all times ?? mask ? change clothes and shower before i
see another baby or my own kids and husband with leukemia in remission ?

2. Can INH cause GI distress in infant ? blood, gas? diarrhea ? fussiness ?
  or is blood due to mixing cows milk base abm and Brmilk ? (these streaks of
        blood went away after she reduced ABM to 2 oz day.

3. Does Depo reduce milk supply when administered so soon after deivery esp
premie ??? any studies and why do these docs do this to new moms

4. Mom c/o instant headaches about 10 minutes into feeding ???

  is this prolactin surge headache ? or oxytocin ? or DEPO ? H/A happened
after MeR about 5 mins later ? what can we do to help ?

5. She has a fennel colic tea from brazil to give the baby is this ok what
else might be in the tea (from brazil)


6. Can any one e-mail me some Portugese breastfeeding wisdom or
instructions that will encourage her to believe she :  Has enough
breastmilk if she would just nurse any time  and count diapers !!!! That
she should hold her baby alot talk to and touch  (she thinks she needs to
do every thing like at the NICU they did SAVE her baby's life after all) 3
hour feeds--pacifier--don't touch--don't use water on the baby--don't bathe
(cords off)--lotions--vaseline--baby wipes is all that touches this kid if
her baby wants too eat more than every 3 hours than her milk is not enough
in her eyes...like the baby ate ABM every 3 hours for 2 weeks  oh and use
the monitor VERY important (Fisher Price not cardiac) she's afraid to sleep
with baby on her chest etc etc.

7.  Can you email me portugese instructs?  what can i do to convince her to
stop so much pumping and try just nursing ? she speaks so warmly of the
time he nurses you can see she's comfortable with it BUT if he acts fussy
or sucks on his fist "he's Hungry my milk's not enough...." i got a 40 ml
intake at breast after 20 minuts switch nursing, 45 mins after he had
nursed..

8. how can i tell her in portugese to trust her ability to mother, comfort
and feed her baby at the breast...she has the usual american non Brfeeding
support "Don't spoil the baby why do you want to do THAT ? (Brf) you can
see the milk in a bottle better...on and on   whew

any one out there speak portugese and can tell me what to look for with TB ?????
