Our current policy is to use thawed breastmilk (donor or mother' own) within 24hrs of thawing. This was per the HMBANA guidelines book that we have, although we don't have the most current copy.  Our Neos are interested to hear if there are other NICUs that use 48hrs as we have heard is also fairly standard?
