I am doing this work at a very large hospital.  It is
located in the health education department.  The follow-up
call not only gets the mothers in for the breastfeeding help
they need but by entering it into the computer a record is
generated for their chart that indicates their responses and
the suggestions made as well as follow-up referrals.  The
information is also readily available to be used for
research on a wide variety of topics.

Any input regarding the kind of information that would be
useful would be appreciated.  We already question the
following topics:
     Babies DOB and time, hospital discharge, breast or
bottle feeding, frequency of feeding, baby's behavior, skin
tone, appearance of sclera, urine output, stooling (# and
color), breast assessment (soft-engorged), nipple assessment
(normal-bleeding).
     Assessment of mother's health - vaginal vs c-section,
incision healing, epis/tear treatment, vaginal bleeding.
     Follow-up appt. scheduled, plans for birth control,
informed about hot line re: breastfeeding, referrals to md,
urgent care, lc or home health nurse.
    The format is open for revision so that research can be
done on lots of different topics.  Any ideas about what else
could be useful?
