I am currently writing a book on recreational drug use during pregnancy and breastfeeding.
Would you and any of your LC friends be interested in providing possible case study vignettes to include in the book?
All information will not include any personal patient identification.
Your name can be mentioned with the vignette, but you can also remain anonymous if you wish.
You can respond to me at: [log in to unmask] Thank you.
The following represent the case situation vignettes I would like to include in the book:

1. Describe an encounter with a pregnant woman who uses medications recreationally. What were the patient’s concerns? What questions did she ask? 2. Describe an encounter with a breastfeeding woman who shows a desire to use medication recreationally postpartum. What were the patient’s concerns? What questions did she ask?
3. Describe an encounter when a patient is unsure if a recreational or illicit medication is something she can take safely while breastfeeding. What did she learn from her pediatrician? Why was she asking about this medication?
 4. Describe an encounter with a woman seeking guidance, but you did not have a clear cut, well referenced answer. Best if this encounter demonstrates the usefulness of the ABM Clinical Protocol #21: Guidelines for Breastfeeding and Substance Use or Substance Use Disorder, Revised 2015. Retrievable: http://www.bfmed.org/Media/Files/Protocols/Guidelines%20for%20Breastfeeding%20and%20Substance%20Use%20or%20Use%20Disorder.pdf
5. Describe an encounter where a woman admits to Schedule I illicit drug (for example: cocaine, ecstasy, heroin, LSD, PCP) use while breastfeeding. How did she feel? What were her concerns? Why did she decide to breastfeed while using this Schedule I drug? If she is aware of the adverse effects, how does she manage to take this drug and breastfeed? Does she separate intake and
breastfeeding by a certain amount of time? Does she use other techniques to get rid of the drug? How did she hear about this technique? How was the infant affected?
 6. Describe an encounter where a patient admits to marijuana use while breastfeeding. How did she feel? What were her concerns? Why did she decide to breastfeed while using this marijuana?
If she is aware of the adverse effects, how does she manage to take this drug and breastfeed? Does she separate intake and breastfeeding by a certain amount of time? Does she use other techniques to get rid of the drug? How did she hear about this technique? How was the infant affected?
7. Describe an encounter where a woman is unsure if an illicit medication is contraindicated while breastfeeding. What was her course of action? What alternatives did she use?
 8. Describe an encounter where an infant experienced (Neonatal Abstinence Syndrome (NAS) due to opiate or other drug exposure. What did the infant look like? What was the course of action? How did the mother respond?
9. Describe a case where a breastfeeding mother is being treated for her addiction. Why was she using this recreational drug? How was she treated? What alternative therapies were used?
How was the infant affected? With what method and/or drug was the infant treated?
10. Describe an encounter where a breastfeeding mother who uses drugs recreationally is counseled. How did she respond? Did she decide to discontinue the recreational drug use, or discontinue breastfeeding?
11. Describe a case where a breastfeeding mother who used drugs recreationally recovers from
the addiction. What were the struggles? How did she overcome the addiction? How did she continue breastfeeding, or what was her course of action? Frank J. Nice, RPh, DPA, CPHP
