I'd want to take a look at her, and how she fits into the pump flanges.
We've been using more and more of the Medela "Personal Fit" flanges, finding
poor fit to be a common cause of pumping problems.  At our conference last
week I was fascinated to hear Barbara Wilson-Clay talk about measuring
nipples and finding in her small sample of mothers that about 10% (I think
she said) had nipples too big to fit into standard-size pump flanges.  Maybe
it won't turn out to be that high, but I know there's a significant
percentage out there.  Perhaps this isn't the problem with your mom at all,
but if the nipple is just rubbing in the nipple tunnel of the flange, she
wouldn't ever be able to empty her breasts.
