This is the sort of situation that is just made for using a supplementer.
The mom and baby get the nursing relationship, and baby gets some
breastmilk, for as long as possible.
Although the occasional mom (with apparently insuff br. tissue) has no milk
supply problems, not enough milk is usually identifiable early on and a lot
of exhaustion and grief can be avoided by being up front and honest and
suggesting a supplementer. I've found it works both ways, some moms have
been just relieved at the explanation and switch to a bottle and others are
delighted with maintaining the nursing relationship and using a
supplementer.  I think these moms have a pretty good ( perhaps deeply hidden
tho) idea that something is radically wrong with their breast shapes and
sizes.
