I take my hat off to your generosity as I don't know of many docs (at least in
the UK) who would think about anything like bras.

Masititis often begins with less than optimal drainage.  Bras can affect
drainage.  My first place to start (and I hope no one mentioned it because it is
*so* obvious we would all do as a matter of course) would be:  Check the
positioning and attachment of the baby at the breast.  Sometimes it looks ok.
Still worth checking to see if it can be changed in order to improve the baby's
ability to drain all the milk effectively.  As this case has had an occurance in
both breasts, it suggests drainage to me.

Also -- it is best if you can see the woman feeding on her home territory.  I am
always amazed to see so much furniture in the world which is so poor for easy
breastfeeding.  Some day I will get around to lecturing sofa manufacturers......
