This situation sounds a lot like Overactive Ejection Reflex (of Hindmilk
Foremilk imbalance,) which can be aggravated by scheduled feeds.  I agree
that a new doc for this mom is in order. Along with feeding on demand while
using one breast for a 3 to 4 hour period and removing the baby during
ejection.
