Is this little guy getting Sensory Integration Therapy from an
occupational therapist?  If not, that might be a first step in reducing
some of his issues.

As for the breastfeeding, have moms try making it a game.  I've had
success with older babies in my practice lately (sparked by something
Diane Wiessinger said once) with mom playing with the baby with toys,
and then with her breast, then just happening to move it toward their
upper lip.   One game- touch a soft toy to baby's belly and make a funny
noise.  Then touch the toy to baby's cheek, make same noise.  Touch toy
to nose, etc.  Then do the same with the breast.  See if baby will take
the breast into his mouth of his own volition when the breast touches
his philtrum (that little ridge between nose and mouth).

Mom can try non-traditional positions for this, kneeling over baby seems
to work well, since it's not already associated with feeding.
