I have worked with four women with pituitary adenomas,  three of whom  
were treated with bromocriptine at some point before they became  
pregnant.      None of them produced a full volume of milk even though  
they were treated with domperidone.
  My theory and it is just a theory is that milk production is on a  
sort of prolactin thermostat.   If you always keep your home  
thermostat set at 30C you will need to reset it very high before you  
start to sweat---if you always set it at 15 you'll notice it very  
quickly if it is reset to 30.    If your body is used to having a very  
high amount of prolactin , perhaps you just can't raise the levels  
enough to produce large amounts of milk.
There maybe very many women with pituitary adenomas who have  no  
problems with milk production but those aren't the women that I see.
