"This particular physician is being very difficult, saying that my
identifying this and suggesting the mother have it evaulated is "never to
happen again!"  She stated I am out of line in my practice, and this
procedure is "outmoded" and "unnecessary."

Is this physician your supervisor? Or do you work for her - i.e., does she
pay you?

I can't imagine where she gets off speaking to you that way! You don't tell
her what she can & can't do in her practice, do you? I can just picture
myself calling up some practioner who's said something stupid like "your
baby is allergic to your milk - you'll have to switch to formula" and
telling him or her that "this is never to happen again"!!?

While your practice as an RN & IBCLC is different than hers, it certainly is
not "less than" hers. Can you find a respectful and non-combative way to
discuss it with her? I know there are lots of things in my professional life
I've just had to put up with, but this kind of comment is designed to put
you on the defensive, and doesn't work to anyone's benefit. (Least of all
the baby & mother you *both* should be serving! Hey, at least you were
right! Not that you'd want to bring that up, of course...)

Sheesh!
