I second many of the excellent recommendations (Atlas, BAB, 
Breastfeeding Management for the Clinician, Supporting Sucking Skills, 
The ABCs) and wanted to add "Counseling the Nursing Mother" 4th edition, 
by Lauwers and Swisher.  I borrowed a copy, without any big 
expectations, and (though not finished with it yet) have found it rich 
with lots of practical information, both on the mechanics of lactation, 
and on counseling skills.
