Have you tried suggesting to this Mom that she feed off of only one breast
per feed?  I've had some great results with mothers that put baby to the
same breast every time they want to feed during say a 2 hour (1.5-3hr?)
period.  Their breasts seem to adjust to the routine within a couple of
days.  It seems that switching from side to side stimulates the breast to
produce more and more milk and the baby doesn't get very much of the calorie
rich, satisfying hind milk.   I hope this helps your client.
