This is common with a tongue-tie. I would also be interested in the mom's birth as unresolved structural trauma could be implicated.

"The age old complaint--except this one says soreness has existed since =
the beginning. Baby is now 3 months old.  No yeast, she's tried lots of =
advice from everybody except a lactation consultant!   I met with mom =
and baby last week and observed a "chewy" kind of latch by the baby.  =
Lips look good, but once the letdown happens, he begins to pull and =
chew.  It helped a little to pull him in a lot closer, but the nipple is =
creased, and looks bruised after nursing.  Mom is in a lot of =
discomfort, and has just now given him his first bottles since she is =
beginning to dislike breastfeeding--despite being quite devoted.   Her =
description of the pain does not sound like Reynaud's since the cold or =
temperature changes do not make it much worse.  Does not fit yeast or =
infection criteria.  Anyone with any ideas before she quits.
