Fiona - I have seen this kind of thing I think, but not for this long.  It
seems as if the baby needs an inordinate amount of stimulation to the palate
before she can "latch" and suck. I've seen babies doing this with bottles
too, mouthing around "looking" for something to suck on, getting really
frustrated.  The teat needs to be pointed right up at the palate.   If baby
can suck on a finger (huge amount of stimulation, you can tickle the palate
until the latch occurs), then I would go with pumping and finger-feeding EBM
with a dropper beside the finger (in the corner of the baby's mouth) for a
day or so, or however long it takes for baby to receive sufficient intake to
calm down and possibly really LEARN that she can suck! Then attempt the
breast again. Please keep us posted.
