The shorter, concise version is Protocol #10 for Near Term Infants 35-37 weeks, from the Academy of Breastfeeding Medicine, <www.bfmed.org>
There are references at the end of each Protocol that support their recommendations.

The much longer, much more detailed version is Nutritional Support for the Very Low Birthweight Infant, Toolkits #1 & #2, available from the California Perinatal Quality Care Collaborative (did I get that right?) at
<www.cpqcc.org>
And they have added a number of important new Toolkits since I last visited their site.
