Another possibility is, this is extra mammary tissue. Especially if it's right IN the axilla and mom has difficulty letting her arms down at her side. It's like an extra breast with no nipple outlet. It is not connected to the breast/nipple below it.
The usual antiinflammatories that have already been Rx'd for mom, plus ice packs &/or cabbage until the engorgement goes away are treatments to consider. Lymphatic drainage therapy will probably help this happen faster. If this is extra breast tissue, she can expect the same thing with every baby.
