One practice I see often is mothers of discharged premies being told to add
powdered Neosure or other premie 22kcal/oz formulas to their pumped
breastmilk and given as supplement.  Last week, I saw a mother of a 34wk EGA
infant (at 5 wks of age was near his original due date & had gained weight
quite well) who was still adding Neosure to twice daily pumped milk (giving
the 1-2oz of milk in a bottle just for the added formula). This mother had
never been told to use hot water as the diluent to kill pathogens. And while
I hear the arguments for human milk fortifiers in tiny ones, I can't quite
get the rational of adding powdered formula to breast milk, I wonder about
the osmolality, & I question especially why it would be necessary for
infants who are getting their intake effectively directly from the breast.

So, that reminded me to ask lactnetters (and especially our neos):
Under what circumstances and for how long is it recommended to add Neosure,
etc to breastmilk?  If adding it to breastmilk, what concentration?
And are NICU's adding powdered formula to hot water to kill bacteria?
