If I remember correctly, there is a joint publication put out by AAP
and ACOG with standards for maternity practice.  In it it states
that it is NOT necessary to separate mom and baby for FUO (fever of
unknown origin).  I have had to fight that battle on several
occasions.  My hospital had a policy that if mom had a temp at or
above 100.4 on 2 occasions 4 hours apart, she must be separated from
her baby (including, of course, separated from breastfeeding) until
the temp was below 100.4 for 24 hours!!!  They could be together
with the Pediatrician's OK.  Most would readily give the OK, but one
was very stubborn about it even though he was very supportive in
other aspects of breastfeeding.  Even when I pointed out the
standards, his position was he'd rather be "safe than sorry".
Fortunately, this separation policy has gone by the wayside, but one
or two docs still resist.  Fortunately, all but one will be retiring
out of the picture in the not too distant future-just hope they
don't "contaminate" any of the younger ones with their opinions
before they leave.
The vast majority of maternal infections (if that was the cause of
the fever) turn out to be incisions etc. and the only precaution is
keeping baby from direct contact with any contaminated materials.
Even when it turned out to be respiratory, most docs still kept the
nursing couple together as long as mom wore a mask and practiced
"good handwashing techniques".
