Susan,
Have you read Nancy Williams' article in Consultant's Corner,  JHL of
earlier this year?  She discusses some cases in which the inhibition was
apparently pschologically related. I've also seen this, and when nothing
else is apparently wrong, I start asking questions about the mother's
feelings, background, fears, etc.
