I don't know about the second question, but, with the help of several wise
Lactnetters a couple of years ago, I was able to help a mother whose baby
(3 months old)was diagnosed FTT and had a battery of tests at Children's
Hosp, where nothing wrong was found.

She did an elimination diet with dairy as the main factor and baby began to
gain weight. The Lactnet "advisors" said that the allergy was interfering
with nutritional uptake. Mother and doctor (and, of course, baby) were
delighted. Mother sent me a picture of him when he was about a year old
and one would never have suspected that he had had weight problems. He
began his weight gain before he began solids.

Since that time these steps have helped other clients.

If a diary allergy is a GI irritant there could be intestinal capillary
bleeds in the infant causing anemia. I have had no direct experience with
this.
