<< I hope someone can help me with a mom who is having sore nipples. She has
given me permission to post to the list. She has a one day old healthy
baby. Her nipples are inverted, but become erect with stimulation. Her
breasts are small and firm. Her areola is also small.

The baby latches on with flanged lips, covering about 1" beyond the
nipple. The tongue cups the underside of the nipple. I can see great
areolar compression and hear swallowing. The problem is that mom is in
severe pain, and her nipples are pinched when the baby comes off. They are
already getting pink and starting to crack. >>

Joyce,

Have you tried prone position to provide a deeper latch? Does baby possibly
have bubble or high palate? She could use Jack's ointment or hydrogel
dressings and I wonder if she needs to air dry her nipples thoroughly in
erect state so when they retract after feed at rest they're not wet (straight
from Barbara Wilson-Clay's BF Atlas). I've had moms with taut breast tissue
make sure they shape their breast in sandwich technique so baby can grasp
this inelastic tissue better; it is hard for them to make a teat when breast
tissue doesn't "give" a bit. Maybe she could also express a little milk and
use of tubing and syringe at breast in the beginning of feed could entice a
deeper latch. Good luck!
