You may want to look into cultural practices regarding prenatal (and
postnatal) preparation of the nipples.  Another possibility is feeding of
teas or other drinks to the babies in the immediate postpartum time causing
nipple confusion/chomping/sore nipples depending how it is given (ie
bottle).  If you follow a trail - toughening nipples > small
fissures/abrasions > bad latch > sore nipples > mastitis.

Just a thought.
