Dear Colleagues.  

 

I would appreciate your input with mother and baby breastfeeding relationship

 

I was asked to see this mother who delivered a 8.11 oz baby boy. Baby lost weight initally but now weighs 9lb .3ozs she is now 10 days postpartum and has mastitis in her left breast. Has been on Flucloxacillin 500mg QID for 24 hrs . Volatren 75mg BD & Panadol 1gm 4 hrly.  

My advice was to continue to pump 3 hrly through the day and 4 hrly overnight.  Has been using a hopsital grade medela pump. 

Baby hasnt been put to the breast for 48 hrs  because of the pain due to chewed, cracked and bleeding nipples and the mastitis 

To-day had a call from her partner to say that she has mastitis in her armpit of her left breast. temp 39. and she now wants to stop breastfeeding She was  admitted to hospital for IV therapy then discharged home on higher dose of antibotics.

 

I have suggested that abrupt weaning isnt the answer - and advised her to continue to pump 3 hrly through the day and 4 hrly overnight until the symptoms of mastitis has resolved. 

then to miss one pump every 48 hrs (as per the breastfeeding answer book)and to pump 4 hrly instead of 3 hrly. 

Unfortuneatly this mum isnt  enjoying her baby or the breastfeeding experience.

 Any input that you can offer would be greatly appreciated.
