Dear Wise Ones,
I have a case in which I not sure what to do.  I haven't had a chance to
examine the woman yet (practicing medicine over the telephone during this
holiday weekend!)  I have her permission to post.  She is currently 36 1/2
weeks pregnant, and is also nursing her 18 month old daughter with a plan to
tandem nurse soon.  The toddler had antibiotics for an ear infection about 1
month ago.  For about 1 week, mom has been noticing increased nipple
soreness on both sides.  At first, she thought it was just pregnancy
related, but now she is noticing that her nipples are really red and peeling
a little bit.  Today, she finally pinned the toddler down and looked in her
mouth, and she sees white patches inside her lips and cheeks.
Sure sounds like thrush, right?  Now - what do a I treat them with given the
pregnancy?  I usually like to use diflucan for cases where mom and baby are
both affected, but I've never used diflucan in pregnancy and couldn't find
anything about it's safety in pregnancy.  Mom and her husband are
pharmacists, so pretty knowledgeable themselves, but they weren't sure
either.  We are trying topical nystatin for mom and baby for the weekend -
but if anyone has any other ideas in case it doesn't work I'd love to hear them!
