I saw a mother today whose baby is FTT: BW 6-15, 2 wks 6-12, today 6-5.
Mother is pediatric RN age 28, history of ovarian cysts and BC pills to
control since age 15, took 1 yr and 1 miscarriage from cessation of pill
to conceive this baby, has been hyperthyroid for the past 10 yrs.
Pregnancy completely normal, minimal breast growth: she still fits 2 out
of 3 prepregnancy bras for 34B. Left side larger than right, but not
abnormally so, no "udder shape", breasts more firm and upright, normal
nipple, 1/2" areola. Baby's suck appears good, goes to breast eagerly,
elicits an MER and swallows 6-8 times, then slows to almost nothing.
Mother's milk supply apparently compromised despite feeding every 2-3
hours for 30-40 minutes. Baby's output has been about 6-8 pale yellow
diapers and 2-4 small yellow stools. Mother says baby has been "fussy" a
lost; when supplemented in office, baby settled down for the "first
time".  Neither of my two major textbooks address hyperthyroidism as a
lactation problem, but I don't know what else to look at here. Any
suggestions? Latch-on wasn't great (mother holds baby up to breast, baby
nibbles on, but I see that a lot with mothers with more upright breasts).
Mom has been double pumping about 5 times a day with Nurture III after
feeding since Monday and taking fenugreek capsules to help boost milk
production.

I'd be happy to fill in any missing info here.
