The book "Adventures in Tandem Nursing" by Hilary Flower (published by 
LLLI) is an excellent resource for a mother who is nursing while 
pregnant.

To add my own personal experience:  I gained 36 lbs. in my first 
pregnancy and gave birth to a 7 lb. 6 oz. baby girl.  She was 13 
months old and still very reliant on breastmilk when I got pregnant 
the 2nd time.  I nursed her A LOT during that pregnancy, gained 34 
lbs. and gave birth to a 9 lb. 4 oz. whopper of a boy.

That boy is going to be 3 next week, still nurses a few times a day 
(ouch), and I'm 28 weeks pregnant -- on track for another 35-ish lb. 
gain.  

"Eat Well, Lose Weight While Breastfeeding" (author escapes me at this 
moment) gives a very good summary of each nutrient and how/whether the 
maternal diet affects its presence in the milk, how the body either 
uses or deals with a dietary deficiency, etc.  For example, a mother 
who is not taking in enough calcium to support the lactation and the 
pregnancy will experience a loss of stored calcium from her own body.  
Nature will provide for the fetus first.
