Thank you all for your responses.
I saw this mom yesterday, and with discontinuing the ointment she was using, and only pumping (not breasting),  her nipples have improved-according to her.  I did not see any "pus" but there was cracking at the bases and excoriation of both nipples.  Mom has nipples  not quite the size of a nickel and baby is not opening wide.  Mom had been pulling down on baby's chin and sort of stuffing nipple in.  She wanted to try a nipple shield she had ordered and so I assisted with that and mom stated it was more comfortable than bare breast but still could not tolerate it for long.  Babe kept popping on and off which made mom wince.  We did a fingerfeeding of ebm and discussed options.
Just  now called to check on her and mom has continued to pump, using hydrogels and ice after pumping and bottling ebm.  Says nipples are continuing to heal, although still having some bleeding from worst side, but not enough to color the milk.  She's not ready to try baby back at breast yet.  She is supposed to call me back tomorrow.
Any more suggestions?

If you look at it and there really is a green drainage, I think I'd get a
culture just to be on the safe side. I've had a couple of staph , etc.
lately develop on moms nipples.

Hello everyone,
I just got a call tonight from a 2nd time bf'ing mom of a 2 wk old
who has had tender nipples from the start. Her latch in the
hospital looked good, but I did work w/ her toward getting a deeper
latch because her Lt nipple after 24 hrs had a red mark diagonally
across the nipple, and was sore. She began using Dr Newman's all-
purpose nipple ointment at that time and has continued it. She calls
tonight to say that this nipple is getting worse and has started to
show "green pus" drainage on her breast pad. Baby also had a fussy
night last night, but has not refused that breast. The other nipple
has improved. She is going to bf on the "good" breast and pump the
other until I can see her on Monday. I've never run into this
before. Any comments?

I just attended a talk by Barbara Wilson Clay. In the case of infected
nipples, she strongly recommends that LCs reconsider basic first aid,
i.e. soap and water cleansing, with mild soap, and possibly oral abx for
extremely bad cases. The nipple should be rested until healed.
