Yesterday I went to see my client with her 3 weeks old baby.  I
posted a story about her already, but I will tell you what I saw.
The baby is well attatched, when the feed was finished, you could
actually see where the mouth had been, it had taken lots of areola
and the nipple had been centred.  However the Mum complained of
nipple pain and pinching continuously throughout the feed.  We tried
reattatching a few times, but it made no difference to how Mum felt.
The nipple looks as if it has been pushed to one side (looks worse on
the right nipple) and the breast tissue looks as if it is
occasionally being pulled to one side when the baby is sucking.  The
tongue is forward on attatchment and at times during the feed, but
sometimes it would disappear during the feed.  There are no
anatomical problems in babies mouth.  Mum has flattish nipples.  She
has taken away the dummy with no noticable difference to the feeding
as yet. Baby feeds well at night, mum has no nipple pain.
I suggested suck training, changing positions and if in a few days
all else fails, a nipple shield.  I really would appreciate it if any
one thinks they can offer me any other tips or thinks I am on the
wrong track.
