The heat is applied over clothes immediately after nursing or pumping ...not  
before...and left on for at least 5 minutes.

I have used Niphedepine once or twice, but heat usually works very well and  
has no side effects.  And assuming the source of irritation has been  
removed, it only takes a few days of heat after EVERY nursing or pumping to  
stop the spasm.
