I wanted to reply to this because I worked with someone recently who turned out to be having significant vasospasm (Raynaud's of the nipple). 
However, it didn't show itself immediately and all other clues pointned directly to yeast/thrush. 
I think it was the 3rd or 4th time I saw this particular baby feeding that I finally witnessed the vasospasm.

I thinking it's worth considering, especially if there is a possible tight frenulum and high arch to the palate, those could potentially aggravate vasospasm.

I have also seen moms have significant bruising from untreated tight frenulums and with this have seen pain persist for quite some time ... am always amazed at those who persist through the long lasting pain to get to a point where nursing is enjoyable.
