I have had very good success with the improvised version of the SNS (Let's
not even begin to get into the discussion now of "using a product for
something else").
I have used it mostly for cases where the mother needed to supplement short
term becasue of insufficient weight gain/poor supply.  When I show the
mother how to use it and she sees what it really should look like when a
baby is getting a mouth full of milk at the breast when there is good flow,
compared to what the baby did before using the device (usually ineffective
shallow sucks with falling asleep after a short time), I have found it very
helpful.  It is very important to use it correctly:  First feed without it,
with breast compressions; making sure the baby finishes what is in the
breast; also having the mother work to build her supply while supplemeting
with the device.
Obviously it is not useful if the baby is refusing the breast, and the baby
needs to be latched on well for it to be used most effectively.
I use it as described in Jack Newman's handout "Using a Lactation Aid", and
I have had mothers very satisfied with the results.
(I also have translated this handout to Hebrew and I give it to the mothers
using it).
I find that we convey our attitudes to the mothers.  If we think it is
cumbersome, complicated  and ineffective, it is unlikely we'll be able to
get any mother too enthusiastic about using it.
