Was the suggestion to pump until letdown occurs made because baby begins to
sputter and choke when the letdown occurs?  Or because the baby is anxious
for milk to start gushing out as soon as he/she latches-- like it does with
a bottle? If the former, I'd wonder about tongue tie or other sucking
problem, if the latter then getting the breast stimulated to the point of
let-down would give him/her the instant gratification he/she needs.
