The case regarding the 24 y.o pregnant woman with lymphedema who's breasts grew to basketball size reminds me of a case I had a few years ago.  As a hospital lactation consultant I visited a mother who had one breast that grew rapidly and out of control (at least 10 times it's normal size) during her pregnancy while the other breast responded appropriately to the pregnancy.  Her normal size was a 34b but the enlarged breast grew from her 3rd month becomming hard, swollen and painful.  She was unable to obtain any milk from pumping and due to the tight "engorged-like" breast, the baby could not latch.  I feared breast cancer and notified her OB.  After discharge, the mother saw a breast specialist who did an ultrasound which determined that although she had glandular tissue there were no nipple pores/openings that would allow the milk to drain.  
Since it was mentioned that the mother Loni saw also experienced no leaking, perhaps this mom could talk to her physician about having an ultrasound to rule out whether her breasts have patent nipple pores.  The mother I saw gave permission for her picture to be published in Jan Riordans photo CD located in the back of her latest text book.
