Sounds like you did really well in a really horrible situation.  You didn't shrink from the evidence, nor from your ethical obligation to act as advocate for this dyad.  Nice one!  Reading your story, I wondered if maybe this nurse needs to be reminded of the concept of 'informed choice'; of her ethical obligation to respect a woman's wishes with regard to her body and that of her infant.  The mother is perfectly entitled to refuse (any) recommended treatment for herself or her baby.  Whose baby is it, after all?  
Knowing the tight spot that nurses often find themselves in, I do wonder if the nurse was concerned about how she would explain the failure to supplement to the doc.  Nursing ain't easy, either.
