We do not have an official policy on KMC in our newborn nursery, but we do 
encourage it, especially when we (the LCs) get calls from the NN nurses about 
sleepy babies. If the baby has gone 4+ hrs and hasn't BF then we encourage 
KMC for 30-60 mins before attempting to rouse the baby again. This has 
worked so often for us that the NN nurses now buy into it. 
The NN does have a policy that if the baby hasn't fed for 6 hrs they must 
check the blood sugar, and if low, follow protocols, but usually the mention 
of "sticking" the baby is enough to motivate moms to wake baby and BF.
As for co-sleeping, we don't have a policy against it, only that if mother has 
had a sleeping pill, sedation, or is too tired to wake if there is a problem, then 
someone else must be in the room at night.
I frequently walk into mothers' rooms to find them both fast alseep in the bed. 
Usually baby is curled up besides mom in the crook of her arm - it's close 
enough to KMC for me :)
