Mother should have all of the support that any other mother would recieve in establishing her milk supply, early pumping or expression of milk, regular milk removal and care.  The timing regarding when baby will be able to go to breast will be dependent upon the extent of the hernia and the baby's post-surgical course. Some babies require assisted ventilation for a period of time following the surgery.  Different units have varying policies regarding how soon oral feedings are started.  some will start tube feeding before the baby is able to suckle.  If the mother has colostrum/milk stored it can be used for these feedings and the baby benefits from the anti-infective, anti-inflammatory factors in the breastmilk.  On occasion these babies may develop a kylo-thorax and that is a whole different scenario which I think has been discussed on LactNet in the past.

Hello

We have a pregnant mother, near term. Her baby has a diaphragmatic 
hernia, and a aurgery is planned immediately after he is born. This 
mother want to breastfeed. Has someone of you had such a case ? Have 
suggestions to help this mother ?
