A comment from Finland, Scandinavia.
On the Finnish breastfeeding support mailing list this subject is brought up on a regular basis. Usually the mother linked the cause to allergies, sometimes the reason was found to be a bug.

Here the vit D drops are recommended starting from age of two weeks (till three years). Remember, we are quite up in the north and there is no vit D syntesis on the skin from Oct till March.
