Try positioning the baby in the *exact* position on the opposite breast by sliding him over.  i.e. no football hold--if baby lays in the cradle hold on the right, make sure the angle is exactly the same on the left.  Mom can try to wrap the baby around her opposite side, or just have a stack of pillows next to her to be the auxillary lap.  And, of course, refer.  Where I live, I'd refer either to the doctor or into the Infant-Toddler Intervention Program.  They will do a free courtesy screening and fax a referral to the doctor.  (I like this idea because the people who specialize in the problem are screening for it)
