Two weeks without healing is worrisome. I would consider bacterial infection
(possibly staph) as a likely culprit, possibly needs topical antibiotic
treatment, possibly with an APNO type ointment (antifungal and cortisone
included), and potentially needs systemic antibiotic therapy. Good and
gentle soap and water cleansing of the wound several times daily would be a
good idea, in my opinion.
I would also consider Paget's disease of the nipple, a form of cancer.
I would refer this mother to a primary provider for these evals/treatment,
and I would followup.
