So my question is:  How much Domperidone is too much?  I just spoke with a
mom today (I am NOT her LC) who told me she has been taking 160mg of
Domperidone per day for months - her son is now 14 mo old.  She said that
whenever she tried to drop down to the 80 mg per day her supply dropped with
it - but at 160mg/day she can maintain a full supply.  Thoughts?
 
 
Also, along the lines of low milk supply:
 
Back in February I talked to a mom who was battling low milk supply issues.
Her situation was:
 
Infertility issues - tried 1.5 yrs to get pregnant - One round of Clomid and
got pregnant.
 
History of chronic annovulation.
 
She averages 30lbs overweight (not sure if that would be considered obese -
she is 6'1")
 
Family history of thyroid and diabetes (Mother)
 
Breast changes in pregnancy, sore nipples, breasts got fuller but not a full
cup size. 
 
Mom was anemic during pregnancy (not sure how anemic - she was told she had
low iron levels and was put on iron supplements).  
 
Baby born 1-26-06 - NO changes in breasts until day 4-5.  Delivery was vag -
no meds, no issues with baby.  He was 8'13"   (currently 10wks old - don't
know what current wt is)
 
Saw another LC at that time (baby approx 5 days) - per pre-post wts no milk
intake at breast - SNS started.  
 
I suggested a prolactin check - Day 19 her levels were 154 - which,
according to Lawrence were right where they should be to be producing a full
supply.
 
Mom started Domperidone - takes 80mg/day (minimal results) and More Milk
Plus Special Blend.   Has been on both for approximately 7 weeks.
 
Herbs and Domperidone not doing anything much.  We discussed Hypoplasia and
I suggested she talk to her Dr or LC to rule it out.
 
I met her in person today (by accident at an Infant Massage class I was
teaching and she happened to show up at!) and we talked after class.  She is
frustrated - 
 
Currently - Per pre-post wts baby takes in approx 1oz of breastmilk (from
both breasts together) per feed - nurses at breast 8-10 times/day.  Nothing
much has changed - no real results with the Domperidone or herbs.  She is
seeing another LC in her health care system next week.  I suggested she talk
to an endocronologist to rule out pituitary issues.  She asked if I could do
a visual exam to see if she might have hypoplasia.  Visual exam showed
somewhat wide spacing between breasts (3 finger-widths) but otherwise fairly
"normal" appearance.  Not what I have seen with other moms who do have
hypoplasia.  I'm at a loss and mom wants answers.  Is this hypoplasia that
doesn't really look like it or is it an underlying hormonal issue that drops
the milk supply even when the prolactin levels are high?
 
Thoughts?
