I would explore the possibility that the 3 yr old has undiagnosed pin worms.
Here in Texas, parasites of all varieties were an issue when my kids were
little.  They were always contracting something or another:  Ring worm, head
lice, pinworms, ticks.  And it seemed I spent my life cleaning!!  We had a
lot of animals and my girls played outside a whole bunch is my only
explanation. God only knows the awful stuff they put in their mouths.
Probably only the fact that they all breastfed forever kept them alive.

 One of my girls had a spell of an uncommon number of vaginal infection
problems as a toddler, and finally the doctor thought to test her for pin
worms.  Once she was dosed, she quit having vaginal discharge and itching as
well as fitful sleep.  This same doctor once told me:  "Show me a kid whose
never had pin worms and I'll show you a kid with no friends."  His point
being that at that age kids are always exchaging way too many goodies with
their buddies.
